Partnership to pilot new style of care for older people

Barfoot & Thompson is excited to announce a new partnership with the Well Foundation, the official fundraising body for Waitemata District Health Board (DHB).

For the first year, the annual contribution from Barfoot & Thompson is helping to fund the CARE Project. 

The CARE Project (Coordinated care, Assessment, Rehabilitation and Education) is a pilot programme with Waitemata DHB to coordinate and enhance the care of older people living in their homes in our communities.

Barfoot & Thompson will support a team of gerontology nurse specialists, who will be responsible for training and education in the general practices involved in the pilot.

About the CARE Project

The pilot will target patients aged 75+ years and Maori/Pacific people aged 65+ years, providing a comprehensive package of care, integrating primary GP care, hospital services and non-government organisations to keep older adults healthier in their homes for longer and less likely to need hospitalisation.

The project involves general practices in Hibiscus Coast, Whangaparaoa Peninsula and Helensville, with other general practices in the Rodney/North Shore area likely to be included next year.

The gerontology nurse specialists will initially work out of five general practices and will play a key role in training and supporting staff, with a focus on instigating proactive, rather than reactive care.
